Getting Started

Once you are ready to start your physical therapy regimen, contact MVP to schedule your initial evaluation appointment.  You can expect the initial evaluation appointment to take approximately one hour to an hour and a half depending on the type of injury.  Your subsequent appointments will typically take about an hour.

Once you have begun your physical therapy regimen with us, you are welcome to use MVP’s professional gym / training facility for working out.  This is a great bonus to doing physical therapy at MVP!  You are also welcome to come in on you off days to use MVP’s gym / training facility.  However, these options are limited to patients 18 years of age and older.  MVP does offer programs for youth patients – please inquire with one of MVP’s staff members.

What to Expect

On your first visit, you will need to complete your intake paperwork, which should take about 5 to 10 minutes.  For this reason, we ask that you arrive to your first appointment  about 10 minutes early.  You will also need the following:

  1. A form of Identification
  2. Your Health Insurance Card (if we will be billing your insurance)
  3. Your Prescription (which is typically required by your insurance)
  4. Comfortable Clothing (like what you would wear to workout)
  5. Shoes (like what you would wear to workout)

Do you need a prescription for your doctor to fill out?  Click on link below, print it out and take it to your doctor.  You have a choice of where you do your physical therapy, so make sure you choose the best!
